This post explains my approach to supporting patients who have chosen to discontinue topical steroids and are navigating the withdrawal period.
Topical Steroid Withdrawal (TSW) is a complex and often distressing process for many individuals. Most patients arrive already fully off topical steroids; but there are others who are still tapering or deciding what to do next.
This is not a recommendation to discontinue medication — it is information for individuals who have already made that decision independently and want to understand how herbal medicine may support them during the transition.

A Personal Decision: Staying on Steroids vs. Stopping
Whether someone should stay on topical steroids or stop is a personal medical decision that I do not try to influence. I only prefer that individuals have a clear understanding of the pros and cons of each approach as there are valid reasons why different individuals might approach the situation differently:
Reasons some individuals stay on steroids and conventional treatment approaches:
- Their symptoms rebound intensely soon after stopping and they do not wish to experience the flare up
- They are not ready emotionally or physically to handle the withdrawal period
- They prefer non-steroidal prescription options (e.g., tacrolimus, crisaborole, ruxolitinib)
Reasons someone may choose to stop:
- They’ve experienced diminishing returns
- They have concerns about long-term use
- They believe their skin worsens after stopping
- They suspect symptoms are related to withdrawal rather than the original condition
I see my role as supporting patients who have already made the decision — not to push them toward it.
Why 2–3 Weeks Off Steroids Are Required Before Treatment
Whether a patient will experience a brief steroid rebound or begin developing full topical steroid withdrawal (TSW) can only be determined once the skin has been off topical steroids and other non-steroidal topical prescription drugs for a period of time. For accurate diagnosis and treatment planning, I require a minimum of 2–3 weeks without any topical steroids or prescription creams before starting herbal therapy.
What This 2–3 Week Window Allows Me to See Clinically
This time frame is not about reaching a new baseline. In fact, many patients would continue to decline if left untreated. Instead, these early weeks allow enough clarity to see the true underlying direction of the condition once pharmaceutical suppression is removed.
TCM dermatology works for TSW because treatment is individualized. To tailor herbs correctly, I need to assess the actual, unsuppressed presentation of the skin and the overall systemic state. This includes evaluating:
- the degree and distribution of redness
- dry, flaky, weeping, or crusting areas
- involvement of the face, neck, ears, scalp, groin, and underarms
- presence and frequency of weeping
- nighttime itching or itch attacks
- warmth or cold sensations (whole body or localized)
- how much skin shedding is present in the morning
These findings determine the herbal strategy, category, and dosage.
Why This Stage Can Be Emotionally Difficult for Some Patients
This period can be understandably difficult. For some, symptoms worsen quickly after stopping steroids, and that can be emotionally challenging. But this pattern is exactly what I need to evaluate so I can treat the underlying factors appropriately.
To be clear in a grounded and respectful way: I have seen very positive outcomes in patients who come to me after deciding to discontinue topical steroids. However, attempting to start herbs first and then stopping steroids later does not reliably prevent the rebound reaction and worse the false hope seems to make the scenario more emotionally challenging. But just as important is that treating TSW with Chinese Medicine is very possible, but seems to work best at least 2 – 3 weeks following the last dosage of topical steroids, oral steroids and/or other topical prescription treatments.

Building a Clear Foundation for Herbal Treatment
For those who have already chosen to discontinue topical steroids, this 2–3 week observation window creates the foundation for an effective, individualized herbal treatment plan.
When Herbal Medicine Can Help
After this 2–3 week period, herbal medicine can support:
- reducing heat and inflammation
- easing itch and improving sleep
- decreasing oozing
- calming large-area redness
- supporting skin-barrier repair
- regulating underlying constitutional factors
The herbal formula is customized for each patient based on the precise presentation.
What Herbal Medicine Cannot Do
Herbal medicine cannot:
- reverse symptoms overnight
- reliably prevent all symptoms of a rebound effect post-steroids
- eliminate all discomfort in the early stages
Setting realistic expectations is essential for a stable treatment journey.
My Results With Patients Who Choose This Route
For Patients who:
- have fully discontinued steroids and other topical prescription treatments
- complete the 2–3 week observation period
- follow their herbal plan consistently
often experience meaningful improvements, including:
- decreased heat and/or specifically heat at night
- decreased redness
- more stable skin
- fewer flares
- improved sleep
- better moisture balance
- reduced oozing
Every case is different, but many patients do very well once the treatment aligns with the true pattern of their skin, meaning not how the skin presents with the assistance of topical medications. Improvements in heat, sleep, and minor changes on the skin can be seen in 2 – 4 weeks in some cases. With overall treatment lasting between 5 – 12 months in most cases, with the goal of a significant remission of lesions and affected areas.
